The Leadership Circle Profile (LCP) provides 360 degree feedback on leadership dimensions that correlate with successful leadership as well as on dimensions that can get in the way of successful leadership. In addition, raters can provide free-form comments about aspects of a leader's helpful and unhelpful behaviors. I am certified to debrief the LCP.
